Liability / Release Waiver

I understand that participation in Brazilian Jiu Jitsu, submission grappling, wrestling, martial arts, strength and conditioning, and related activities involves strenuous physical activity, close physical contact, and inherent risks of injury.

These risks include, but are not limited to, falls, collisions, takedowns, throws, joint locks, chokes and strangulation techniques, strains, sprains, fractures, dislocations, cuts, bruises, dental injuries, concussions and other head injuries, neck or spinal injuries, communicable illnesses or skin infections, cardiovascular events, permanent disability, paralysis, and, although rare, death.

I understand that injuries may result from my own actions or inactions, the actions or inactions of other participants, differences in size, strength, skill, or experience, the condition of equipment or facilities, or the ordinary negligence of Arrows Jiu Jitsu or its owners, coaches, employees, contractors, volunteers, or agents.

I knowingly and voluntarily choose to participate and assume all inherent and reasonably foreseeable risks associated with participation, whether known or unknown.

To the fullest extent permitted by South Carolina law, I, on behalf of myself and my heirs, personal representatives, estate, successors, and assigns, release, waive, and discharge Arrows Jiu Jitsu and its owners, operators, instructors, coaches, employees, independent contractors, volunteers, agents, representatives, affiliates, property owners, and other participants from claims arising from injury, illness, death, or property damage resulting from my participation, including claims arising from the ordinary negligence of a released party.

I agree to follow the instructions and safety rules established by Arrows Jiu Jitsu and its coaches. I understand that I am responsible for communicating when I am injured, ill, unable to safely continue, or uncomfortable with an activity and that I may stop participating at any time.

I agree not to knowingly participate while suffering from a contagious illness, skin infection, or other condition that could reasonably endanger other participants.

If I am unable to make medical decisions for myself during an emergency, I authorize Arrows Jiu Jitsu personnel to contact emergency medical services and take reasonable actions to obtain emergency care. I understand that I am responsible for any medical expenses incurred on my behalf.

If I am signing on behalf of a minor participant, I certify that I am the minor's parent or legal guardian or otherwise have legal authority to provide consent for the minor's participation. I understand the nature and risks of these activities and give permission for the minor to participate. I agree to these terms on my own behalf and, to the fullest extent permitted by South Carolina law, on behalf of the minor.

I acknowledge that I have read and understand this waiver. I understand that it includes an assumption of risk and release of certain legal rights, including certain claims based upon ordinary negligence. I agree to these terms knowingly and voluntarily.

PHOTO & VIDEO RELEASE

I grant Arrows Jiu Jitsu permission to photograph, video record, or otherwise capture my image, likeness, or voice, or, if applicable, that of the minor participant for whom I am the parent or legal guardian.

I authorize Arrows Jiu Jitsu to use these photographs, videos, and recordings for legitimate gym-related promotional and informational purposes, including its website, social media accounts, advertisements, printed materials, competition coverage, and other promotional media.

I understand that these materials may be publicly accessible and may be shared or reposted by third parties once published. I understand that I will not receive compensation for their use.

I understand that granting or declining this permission does not affect my or my child's eligibility to participate in activities at Arrows Jiu Jitsu.
